ERAD Radiology is a leading privately run and owned medical diagnostic imaging centre with five practices located across Namibia and plans to expand into Southern Africa. At ERAD Radiology, we are committed to protecting the privacy and confidentiality of personal information by users of our website and in our practice.
This notice clarifies how we use or process your personal information and in particular any special personal information that be will collected through your use of our website or information provided by you and on your behalf in the course of interactions with you or those involved in your healthcare either online or in one of our practices.
Any references to “we”, “us” or “our” in this notice shall be to ERAD Radiology; and references to “user”, “you” or “your” in this notice shall be to any person with whom we interact with and whose information we may receive or process. This would include but not be limited to our patients, employees, suppliers, hospitals, ambulance services, referring healthcare practitioners, medical aids, statutory institutions and information technology support companies and auditors.
We are subject to a number of obligations to protect the privacy, security and confidentiality of personal information. We are registered with the Allied Health Professions Council of Namibia, South African HPCSA certified, alongside associated with NAMAF and the MVA Fund.

If you use our website to read, browse or download information, our system may record information such as the date and time of your interaction, the pages accessed and any information downloaded. This information is used for statistical, reporting and website administration and maintenance purposes.
TYPES OF PERSONAL INFORMATION
In order to provide patients with adequate health care services we need to collect and use personal information. It is important to be aware that if we receive incomplete or inaccurate information we may not be able to provide our services that you have requested. We collect information when you voluntarily register with us to receive company related alerts, complete our online booking form or any other query related form.
We may collect, use, store and transfer different kinds of personal data about you including the following:
- Name
- Gender
- Date of birth
- Contact details including phone number, physical address and email address
- Emergency contacts and next of kin
- Payment-related information including credit card details, banking details, Medicare number, concession card numbers and pension numbers
- Communications and interactions with us
- Relevant feedback, complaints and claims
Additionally, in relation to patients:
- Requesting practitioner details
- Results copy recipients
- INFINITT PACS imaging results
- Insurance details including private health fund details and Workcover claim details
- Healthcare identifiers
- Medical history and other health information including but not limited to; imaging history, test results, medical conditions, treatments, allergies, pacemaker use, claustrophobia, implants, medications and use of health services
- Where relevant, family history and lifestyle information, which may include information about your work, relationships, religion, beliefs, ethnic background, sexual preference/activity and genetic information
- Preferences in respect of health services
Additionally, in relation to job applicants and staff:
- Qualifications, skills, experience and character
- Screening checks (including health, reference, background, directorship, financial probity, identity, eligibility to work, vocational suitability and criminal record checks).
- Performance, conduct, use of our IT and communications resources, payroll matters and training.
Additionally, in relation to other healthcare providers:
- Healthcare identifiers
- Referral trends
- IT system details.
Collection and retention of personal information
Personal information will in many circumstances be collected directly from you, for example via patient forms, over the phone or from a face to face consultation.
In other instances, we may collect personal information about a patient from a third party source.
This may include but is not limited to:
- Relatives and personal representatives
- Other health service providers such as general practitioners, specialists, hospitals, day clinics and other medical imaging practices
- Patients INFINNIT PACS record
- We may also collect personal information from the parties to whom we disclose personal information as described below:
The circumstances in which we may collect personal information from a third party source include where the patient has provided consent, where it is not reasonable or practical to collect the information directly and where otherwise permitted by law. This may include where the patient’s health is potentially at risk and his/her personal information is needed to provide them with emergency medical treatment.
We endeavour to store and retain a patient’s personal information securely either using our own facilities or with the assistance of our service providers. This includes:
- In paper based form and other hard copy documents located securely within the practice and at secure storage facilities; and
In electronic records in a secure environment (PACS portal)
